Job Description:
A company is looking for a Telemedicine Physician specializing
in Chronic Care Management.
Key Responsibilities
Conduct virtual exams, document findings, evaluate and diagnose
medical and behavioral health conditions, and recommend treatment
plans
Deliver patient care focused on chronic illness, preventative care,
aging, and hospice care
Develop and review care management plans tailored to individual
patient needs and ensure compliance with clinical guidelines
Required Qualifications
Board-certified physician (MD or DO) from a US-accredited medical
school
Board-certified in Family Medicine or Internal Medicine
Active, non-restrictive medical licensure in a minimum of 5 states,
including priority states such as CA, FL, IL, NJ, NY, PA, and
TX
A minimum of two years of experience as a practicing physician,
with a focus on chronic illness care
No history of malpractice
