Job Title: Home Health Nurse - LVNAveanna Healthcare is seeking a dedicated and compassionate LVN to join our team of healthcare professionals.As a Home Health Nurse, you will be responsible for providing high-quality patient care in the comfort of their own homes. Your expertise and skills will make a significant difference in the lives of our patients and their families.Responsibilities:

• Implementing physician-prescribed plans of care
• Conducting patient assessments and coordinating care
• Administering prescribed medications, treatments, and therapies
• Documenting care provided in our system of record
• Providing health education and training to patients and their families
• Accepting and writing verbal orders from physicians, as required
• Collaborating with the case manager to ensure seamless careRequirements:
  • Active, unencumbered LVN license in the state of practice
  • Current CPR certification from the American Red Cross or American Heart Association
  • TB skin test (within the last 12 months)
  • Six months prior hands-on nursing experience preferred
